Appointment Record

I was going to post these in their own individual posts. Changed my mind...
02/07/11               8 Week Appointment, First Appointment with Liza Rivera.
                              Confirm pregnancy.
03/08/11               10 am blood work.
Jack-or-Jill Test and screening for Downs.
03/10/11               12 Week Appointment with Dr. Schroeder.
Waited over an hour b/c someone had an emergency C-section.
                              Then more blood work afterwards.
04/12/11 17 Week Appointment with Liza Rivera.
Wrote apt down for an hour later with Dr. Schroeder. Squeezed in to see Liza Rivera. Received prescription for Progesterone Injections. Received first shot in series (once a week in buttock).
05/10/11 21 Week Appointment with Titi (CNM).
Ultrasound to measure baby and cervix length.
Baby looking normal. Could not measure some of the organs, so will need to do another ultrasound before next appointment. Cervix length is the same as the first time it was measured.
Recommendations: Modified bed rest. Take it easy. No exercise or long walks. Make sure to eat my normal meals. Baby is measuring average.
5/19 – Phone call from Nurse and Titi. I forgot to get shot two days ago. Advised to get shot asap, then resume on normal schedule next week. Titi submitted paperwork to Baylor to have my follow-up ultrasound (to measure spine and kidneys). Will also perform baseline measurements because of previous history (with Olivia) and to check the placenta (because Olivia was so small last time; just to make sure there are no other underlying issues). We will continue with Progesterone Injections until 34 weeks.
5/26/11 23 Week Ultrasound.
Ultrasound for measurements at Baylor Clinic OB-GYN Maternal Fetal Medicine. Ultrasound by tech, then met with Dr. Kjersti Aagaard-Tillery, MD. Baby looked great, cervix looked great—measured approximately 3.1-3.4. Shared opinion that she wouldn’t have put me on Progesterone as Olivia was not born Preterm—Progesterone should be prescribed FOR previous Preterm Baby, NOT for Preterm Labor Symptoms. Said Baby looked great, and does not think that I would need any more ultrasounds for the remainder of the pregnancy.

06/10/11 25 Week Appointment with Debbie (CNM).
Weight: 125.5
Urine Sample. Blood Pressure. Measured Uterus.
Debbie looked through my “Pregnancy Sheet” for Olivia. Commented that Dr. Schroeder may have done a lot of things “just in case”, but I may not have needed it. She thought the Progesterone shots were a good idea. Reminded that each pregnancy is different—perhaps Elijah’s placenta is attached to a better/stronger place than Olivia’s placenta.
Wouldn’t advise going on California trip in July. However, she would recommend flying (instead of driving); taking a copy of my pregnancy records; research good hospitals nearby just-in-case; do not fly if I’m having contractions; go in for a checkup a day or two before flying.
Talked about possibly taking a Fetal Fibronectin Test. Fetal fibronectin is the “glue” present during pregnancy which attaches the amniotic sac to the lining of the uterus. It shows up later in pregnancy, about 1-3 weeks before labor begins. It is a swab test of the cervical canal for the presence of fetal fibronectin between 24-35 weeks. A positive result is a clue that the “glue” has been disturbed and you’re at increased of preterm labor. I will need to check with insurance to see that they will cover it. Otherwise, will cost $12 out of pocket.
Setup another appointment in 3 weeks. Will do Glucose Test then.
07/01/11 28 Week Appointment with Theodora (CNM).
Weight: 126.5
Urine Sample. Glucose-test Orange Drink. Blood Pressure. Measured Uterus.
Uterus measured exactly right. Theodora was much more optimistic about us taking a trip to California. She recommended taking it easy and resting as much as possible. She recommended doing another ultrasound to measure the length of my cervix. Tim requested she check my cervix digitally. She did and determined I was about 40% effaced and that the baby’s head was quite low (lower than she would have expected). Decided that I should get another Level 2 Ultrasound at the Baylor Clinic. She thinks that maybe my body just carries babies low, which has the baby push on the cervix—not necessarily a result of contractions trying to push the baby out (which seemed to have happened with Olivia…and because I haven’t noticed frequent/consistent contractions). Thinks Progesterone was a great idea. Didn’t think I would need the Fetal Fibronectin test because I’m not having a lot of contractions.
07/07/11 29 Week Ultrasound.
Ultrasound for measurements at Baylor Clinic OB-GYN Maternal Fetal Medicine. Ultrasound by tech, then met with Dr. Stephanie R. Martin. Tech was clueless to most of our questions. Had to convince her to talk to Doctor to get a measurement of the cervix, 2.6 cm. She told us that if I were to have the baby now, he’d be fine…Baylor delivers many high-risk babies at 30 weeks. Sat in waiting room for 30 min b/c the tech and doctor forgot to come find us. Dr. Martin said the cervix length was a good normal length. Suggested taking into consideration of the possibility of delivering a baby in California. If we’re okay with that, then have fun!
07/15/11 30 Week Appointment with (CNM).
Fetal fibronectin test. Received a copy of records to take with me to California (for Chan Reunion). Results returned on the following Monday (negative).
07/29/11 32 Week Appointment with Titi (CNM).
Post California trip! Another fetal fibronectin test. Results returned on the following Monday (negative). Advised to continue taking it easy. No point in checking cervix digitally because I could very easily already be 1cm dilated (since I’m so far along). Titi will be on-call this weekend, so if the fetal fibronectin test came back positive, then she would know how to handle any issues I may have during the weekend.
08/12/11 34 Week Appointment with Mary (CNM).
Weight: 130. Nurse asked basic questions: Epidural? Circumcision? Registered at hospital? Etc.
Everything looking good. Continue with Progesterone shots until 37 weeks. Continue taking it easy.
08/26/11 36 Week Appointment with Mary (CNM).
Weight: 132
Urine Sample. Blood sugar was 1 point over. Since this is the only time my blood sugar has been up (and it’s only 1 point), Mary was not concerned. Drink enough water. Blood pressure was good. Baby measured great. Baby is head down. Heartbeat was strong. No longer need to be on modified bedrest. Encouraged to walk more, light exercise—the baby can come anytime and should be fine. Test for Group B Strep and blood drawn for HIV test.
